Nigella Lawson has made a sweet return to Twitter.

In her first post since June 15th, the day before shocking photos surfaced of her husband choking her at a London restaurant on June 9, the celebrity chef Tweeted an aww-inducing image of a seemingly homemade birthday cake bearing the words "Happy Birthday Bruno" written in icing.

Bruno is Lawson's son from her first marriage to the late John Diamond, a journalist, and the cake ? complete with champagne bottle-shaped candles ? apparently celebrates his 17th birthday.

The Tweet marks the first time the How to Be a Domestic Goddess author has reached out publicly ever since photos showing husband Charles Saatchi, 70, violently placing his hands around her neck went viral. The images drew the ire of the world, as sympathy poured in for Lawson.

Saatchi, a wealthy art collector and global ad agency co-founder, initially downplayed the images as a couples tiff, but later admitted he assaulted his wife, which led to a citation from London police.

Several news outlets, including the UK's Daily Mail, report that a moving van was spotted outside their London home earlier this week, with items including pots and pans and a box marked "cookbooks" being loaded inside the truck.

4K, or?Ultra HD, is the future of television. That's a fairly indisputable fact. It's the same step we took a decade ago to high-definition TV. And like with the very first HDTVs, the first 4K sets we've seen have been expensive. Sony released a screen with a $25K sticker price last year, and even models coming out now generally retail for at least $5,000 when you can find them. Little-known HDTV manufacturer Seiki is challenging this notion with the SE50UY04, a 50-inch LED-backlit set with 4K resolution (3,840 by 2,160?four times that of 1080p) and a price of just $1,499.99 (list).

Editors' Note: This review is based on tests performed on the Seiki SE50UY04, the 50-inch model of the series. Besides the screen-size difference, the 39-inch $699.99 (list) SE39UY04?is otherwise?identical in features, and while we didn't perform lab tests on this specific model, we expect similar performance.

?It's not only the first 4K HDTV we've tested, it's the least expensive we've seen. But when you consider you can get a similarly sized set like the 46-inch version of our Editors' Choice RCA LED42C45RQ?for less than half the price, though, you have to ask yourself just how much 4K content you'll be able to get your hands on now, or even in the near future. If you need 4K now, you won't find it for less, and for this, we've rated this set slightly higher. But be prepared to make some picture-quality and feature sacrifices for all these pixels at this low price.

Design
The SE50UY04??looks very simple, with a thin, glossy black plastic bezel that runs around the entire screen, concave at the edges to give it a sense of depth. It sits on a rectangular black glass base that holds the screen steady but doesn't let it pivot. A light on the bottom center of the screen turns blue when switched on and red when plugged in but switched off.

The back of the panel holds a USB port, an HDMI port, component video inputs, and a 3.5mm headphone jack facing left. Two more HDMI ports, another USB port, a VGA video input, 3.5mm, coaxial, and RCA stereo audio inputs, and an F connector for an antenna face downward. Basic controls sit tucked behind the right edge of the screen, but you'll probably use the 7.2-inch matte black remote the majority of the time. The buttons are small, rubber, and not backlit, but the Menu button and navigation pad are easy to find under your thumb.

Despite its ultra HD resolution, the SE50UY04 is a barebones HDTV. You get no network connectivity, no online services, no 3D, and none of the other extras you'd expect from a 1080p HDTV in this price range. It does support a 120Hz refresh rate, which puts it slightly above many $500 budget sets. The menu system is similarly simple, with very few picture options. This proved to be an issue during testing, since I couldn't adjust the aspect ratio to correct for overscan when the set was connected to a computer. The HDTV cropped the 1080p picture slightly, shaving off a slice from each side. A "dot for dot" aspect ratio also lets you view content exactly as sized, centered on the screen.

Performance
Given its test results, the SE50UY04?should?a have remarkable picture. We test HDTVs using a Klein K10-A Colorimeter, DisplayMate test patterns, and SpectraCal's CalMAN 5 software, and after basic dark room calibration the screen produced a peak brightness of 350.287 cd/m²?and a black level of 0.009 cd/m². These are excellent levels and produce a very impressive contrast ratio of 38,920:1. However, while the panel gets very bright, I noticed that it blew out several of the higher white level squares in the test pattern, indicating that it loses detail in highlights. The 1080p RCA LED42C45RQ doesn't offer nearly as high a contrast ratio (1,796:1), but it showed slightly better shadow and highlight detail.

Color accuracy was disappointing, as seen in the CIE color chart to the left. White was warm and slightly green, while red, green, and blue all missed the mark compared with their ideal values. The chart shows the measured values in circles and the ideal values in the squares, and green and red barely got within range while blue was completely off.

4K vs. 1080p
Because no consumer media currently supports 4K resolution, Seiki supplied us with a media server loaded with native 4K stock video, which looked excellent, even when you were standing right up next to the screen. Footage of cityscapes, bustling urban intersections, speeding trains, and computer-generated action sequences looked incredibly crisp and clear, with far more fine detail than you'd see on any 1080p movie. But that's less a testament to the panel's quality and more to the potential of 4K video technology.

You probably won't buy a 4K HDTV to watch 20 minutes of sample stock footage. You'll want to watch movies and television, and for that you'll need to rely on the SE50UY04's upscaling processor. It produces video that isn't nearly as sharp as the sample, because it has to take each pixel and convert it into four pixels on the screen. It doesn't just quadruple the pixels' size to fit the screen, though. It uses an algorithm to determine the edges and curves of the screen, and tries to determine what each pixel would be at 4K resolution. The result is slightly splotchy video filled with artifacts, like watching a standard DVD upconverted to 1080p. In fact, that's exactly what's happening; in both cases, the screen has to fill in the gaps of the video provided, so pixels and jaggies, which we're used to, become splotches and blurs. This isn't a testament to the SE50UY04's quality, either; even the 4K-upconverting Editors' Choice Oppo BDP-103?Blu-ray player produced similar results when processing video on its end. This is again an issue of the technology itself, not the products.

I watched?Tron: Legacy?on Blu-ray on the SE50UY04, and while the picture certainly wasn't bad, the upconversion didn't make it look any better than on a nearby 1080p screen. Shadow details looked muddled, and made further indistinct with the splotchy upconversion. While this panel can get very dark and very bright, it doesn't offer the subtle steps necessary to really show off sharp objects in dark scenes. For the same $1,500, you could get the Samsung PN51E6500EF, a plasma HDTV that offers not only much better shadow and highlight details, but better colors and features like online services and out-of-the-box 3D.

Power Consumption
For a 50-inch edge-lit LED HDTV, the SE50UY04 is a bit of a power hog. Under normal viewing conditions the set consumes 114 watts, which is high for an LED TV of this size. Because the panel has such a high pixel density, it likely requires more power, but we can't confirm that this is the case. Typically, backlighting is what eats up the most power, with LED HDTVs consuming least, followed by CCFL sets, then plasma. For comparison, the 60-inch?Vizio E601I-A3?LED consumes 118 watts and it's a bigger panel.

Still in its infancy, 4K has a long way to go. And while the Seiki SE50UY04's $1,500 price tag makes it at least feasible to purchase, there isn't enough 4K content to justify it yet. It's a decent screen that gets satisfyingly bright and dark, but color issues and light bloom keeps its performance solidly in the budget HDTV category, where you can find our Editors' Choice set, the 42-inch RCA LED42C45RQ for a third of the price, or the larger and full-featured 60-inch Vizio E601I-A3 for two-thirds' of the price. Unless you're really committed as an early adopter, the SE50UY04 has too many attractive 1080p competitors and not enough 4K media to make it worth the cash.

InterDigital loses first round of US case against Huawei, others

WASHINGTON (Reuters) - Huawei Technologies, Nokia Corp and ZTE Corp did not violate cell phone patents owned by InterDigital Inc, a judge at the International Trade Commission said in a preliminary decision on Friday.

The ITC's final decision in the case is expected to be issued in October.

InterDigital, which buys and licenses patents, had accused the companies of violating seven patents used to make 3G wireless devices. The patents cover conservation of power and ensuring the right data goes to the right recipient, among other technology.

LG Electronics had been named in the original complaint but settled with InterDigital.

InterDigital asked the ITC to ban the U.S. sale of the three companies' 3G wireless mobile devices. While much of the world is moving to 4G, 3G remains broadly in use.

The ITC, a quasi-judicial federal agency, is a popular venue for patent fights because its docket moves fairly quickly and it can order a sales ban for any device which infringes a patent.

Mr. Koontz wanted to build a shopping center in a habitat protection zone. 3.4 acres of wetlands would have been destroyed in the process, although he offered to put 11 acres into a conservation easement in return for getting permission. He sued the district arguing that the proposed options were excessive. Man charged in scheme involving Romney tax returns

NASHVILLE, Tenn. (AP) ? A Tennessee man was charged Wednesday in a scheme involving former Republican presidential nominee Mitt Romney's income tax returns during the 2012 campaign.

The U.S. Justice Department said a federal grand jury in Nashville indicted Michael Mancil Brown, 34, of Franklin, and charged him with six counts of wire fraud and six counts of extortion.

Brown is accused of having an anonymous letter delivered to the PricewaterhouseCoopers LLP accounting firm in Franklin last August, demanding that $1 million in digital currency be deposited to a Bitcoin account to keep some of Romney's income tax returns from being released. The Justice Department said Brown falsely claimed that he had gained access to the PricewaterhouseCoopers internal computer network and stolen tax documents for Romney and his wife, Ann Romney, for tax years before 2010.

A phone number listed for Brown had a message saying it was not receiving incoming calls. He did not immediately return an email message sent Wednesday evening.

The letter said interested parties who wanted the purportedly stolen returns released could contribute $1 million to another Bitcoin account. Bitcoin is a hard-to-trace form of electronic cash.

The Justice Department statement said similar letters were delivered to Democratic and Republican party offices in Franklin.

The letters claiming two decades' worth of the Romneys' tax returns had been stolen came amid last year's Republican and Democratic conventions.

At the time, Romney had steadfastly declined to release more than one year of tax returns, for 2010, and Democrats were trying to portray him as so wealthy he was out of touch with middle-class voters. Last fall, Romney also released his income tax returns for 2011.

Computer thumb drives were delivered with the demand. The letter said the tax returns delivered on the thumb drives were encrypted and more copies would be sent to "all major news media outlets" with the passwords to be released if payment wasn't made.

Relationship expert Tamsen Fadal stopped by The Talk on Monday to offer up some advice to make dating post-divorce a little less stressful.

One of the most important things Fadal advises is to start dating on your own time, and not feeling pressured if your ex enters the dating pool first.

"A lot of times we're paying attention to what our ex is doing after that breakup happens, wondering who he's with, if he's seeing somebody," she said. "It's not a contest to see who gets out there first...You date when you are ready to get back out there and not before."

Fadal isn't the only relationship expert who suggests waiting until you're ready before dating again. Marni Battista, relationship coach and founder of Dating with Dignity, teaches that being able talk about your ex without immediately putting him or her down is a good indication that you are ready for a new romance.

Texas Senate Democrats prep marathon filibuster over abortion

AUSTIN, Texas (AP) ? A sweeping bill that would effectively shut down most abortion clinics across the nation's second most-populous state has stalled in the Texas Senate, and a Democratic filibuster that will only need to last a seemingly manageable 13 hours Tuesday looks like it will be enough to talk the hotly contested measure to death.

After thwarting two attempts Monday by majority Republicans to bring the abortion bill to a floor vote ahead of its scheduled time Tuesday morning, Democrats are turning to Sen. Wendy Davis, D-Fort Worth, to stage the marathon speech.

"We want to do whatever we can for women in this state," said Sen. Kirk Watson of Austin, leader of the Senate Democrats.

The bill would ban abortion after 20 weeks of pregnancy and force many clinics that perform the procedure to upgrade their facilities and be classified as ambulatory surgical centers. Also, doctors would be required to have admitting privileges at a hospital within 30 miles ? a tall order in rural communities.

Although Texas is just the latest of several conservative states to try to enact tough limits on abortions, the scope of its effort is notable because of the combination of bills being considered and the size of the state.

When combined in a state 773 miles wide and 790 miles long and with 26 million people, the measures would become the most stringent set of laws to impact the largest number of people in the nation.

"If this passes, abortion would be virtually banned in the state of Texas, and many women could be forced to resort to dangerous and unsafe measures," said Cecile Richards, president of Planned Parenthood Action Fund and daughter of the late former Texas governor Ann Richards.

Outnumbered 19-11 ? with San Antonio Sen. Leticia Van de Putte missing to attend the funeral of her father, who died last week in a car crash ? Senate Democrats held firm Monday to their razor-thin margin of a single vote to block the bill from moving forward.

That's key since the 30-day special legislative session ends at midnight Tuesday, meaning the filibuster Democrats have promised only needs to last the better part of one day, instead of two.

Davis gave a filibuster at the end of the 2011 session to temporarily block $5.4 billion cuts to public schools, and said she was preparing for her upcoming speech but refused to say exactly how.

She will have to speak nonstop, remain standing, refrain from bathroom breaks or even leaning on anything. Other Democrats can give her voice a break by offering questions to keep conversation moving.

"Democrats chose not to negotiate, and we could not get the block undone," said Lt. Gov. David Dewhurst, a Republican who controls the flow of Senate legislation. He refused to declare the issue dead ? but others were less optimistic.

Sen. Dan Patrick, R-Houston, said the Democrats never should have been allowed to put Republicans "in a box" and complained that many in the Senate GOP were "flying by the seat of their pants."

But the bill's bogging down began with Gov. Rick Perry, who summoned lawmakers back to work immediately after the regular legislative session ended May 27, but didn't add abortion to the special session to-do list until late in the process. The Legislature can only take up issues at the governor's direction during the extra session.

Then, House Democrats succeeded in stalling nearly all night Sunday, keeping the bill from reaching the Senate until 11 a.m. Monday.

The measure only passed the lower chamber after a raucous debate that saw more than 800 women's rights activists pack the public gallery and surrounding Capitol, imploring lawmakers not to approve it.

While supporters say it will protect women's health, abortion rights groups warn the practical effect of the bill would be to shutter most abortion providers statewide ? making it very difficult for Texas women to have the procedure.

Debate ranged from lawmakers waving coat-hangers on the floor and claiming the new rules are so draconian that women are going to be forced to head to drug war-torn Mexico to have abortions, to the bill's sponsor, Republican Rep. Jodie Laubenberg of Spring, errantly suggesting that emergency room rape kits could be used to terminate pregnancies.

In the end, though, the bill passed by more than 60 votes as Republicans and some conservative Democrats approved it.

Still, Legislature rules prohibit the Senate from taking up a bill for 24 hours after it clears the House. Republicans struggled to find a way to break the Democratic roadblock, but the vote swung Monday on Sen. Eddie Lucio, a Brownsville Democrat who voted for the abortion bill when it first passed the Senate a week ago but pledged not to approve suspending the rule with Republicans unless Van de Putte was able to make it to the chamber.

She didn't show and Lucio voted with his party, despite his support for the bill.

If the abortion restrictions go down, other measures could fall with it. A proposal to fund major transportation projects as well as a bill to have Texas more closely conform with a recent U.S. Supreme Court decision banning mandatory sentences of life in prison without parole for offenders younger than 18 might not get votes. Current state law only allows a life sentence without parole for 17-year-olds convicted of capital murder.

Watson said Democrats are willing to pass the transportation and 17-year-old sentencing measures but won't budge on abortion.

"Let's get those up, let's get those out of here," Watson said. "Let's not make these victims of red-meat politics."

Patrick said that if the filibuster succeeds, he hopes Perry will summon lawmakers back for a second or even third special session.

"If the majority can't pass the legislation that they believe is important and the people believe is important," he said, "than that's of great concern to me."

CHICAGO (AP) ? The after-party roared on until well after dawn for the victorious Chicago Blackhawks and some die-hard fans.

Hours after returning from their 3-2 win over the Boston Bruins, the team gave awed fans a close-up look at the gleaming Stanley Cup on Tuesday, hauling their prize around to bars and restaurants around the city for the second time in four seasons.

Just like in 2010, their first stop was at Harry Caray's in suburban Rosemont, where more than a thousand fans were waiting before sunrise in hopes the team would again use the Italian steakhouse to kick off their party. Players took turns lifting the trophy above their heads as fans went wild, still barely able to fathom the two third-period goals just 17 seconds apart that gave the Hawks the win.

"I was in shock. ... I jumped up so high that I stubbed my toe," Frank Espinoza told WBBM radio at Harry Caray's, recalling the stunning pair of goals. "I didn't realize it until the game was over; I felt a pain in my toe afterwards."

The team and their families wound their way through Chicago in a convoy of limos and buses under police escort and trailed by TV news helicopters. At their second stop, The Scout bar in the South Loop area of downtown, team members greeted cheering fans outside with high-fives before filing into the bar. Mayor Rahm Emanuel even showed up to share in the fun, WBBM reported.

Emanuel released a statement congratulating the team on a "hard-fought victory" and offering best wishes to the people of Boston.

Earlier, the team was greeted on the tarmac at O'Hare International Airport with a water cannon salute from about a dozen fire trucks and even more police cars ? all with their lights flashing.

Minutes before the chartered Boeing 737 from Boston touched down at 4:05 a.m., lightning danced across the sky as the edge of a windy storm front passed through.

Veteran forward Michal Handzus, who skated with significant injuries, was the first player to emerge from the aircraft, hoisting the Cup above his head with both hands and shaking it several times. Guests, police officers and firemen cheered at the bottom of the stairs.

Players, coaches and team officials mingled with the crowd for about 10 minutes before heading for the city to continue the party at locations that were kept secret until the team showed up.

"This is completely amazing," said Willis Morgan, a 24-year-old electrical engineer, soaking up the scene of celebration at Harry Caray's.

"I watched the game at work on my phone and then we came right over," he told the Daily Herald newspaper of Arlington Heights. "Having the Stanley Cup back in Chicago is unbelievable."

A parade for the team was planned for Friday morning.

The Blackhawks fifth Cup in their 87-year history capped an amazing run through a lockout-shortened season that didn't start until mid-January.

Chicken in Teen Diet May Ward Off Colon Cancer

Eating chicken during high school may reduce the risk of a precancerous condition that may develop into colon cancer, a new study finds.

In a study of nearly 20,000 women, those who ate more chicken during their teen years had lower risks of developing colorectal adenomas, which are benign tumors that may progress into colon cancer.

The researchers didn't find a direct relationship between red meat intake and adenomas, but the results showed that replacing one serving per day of red meat with one serving of poultry or fish may reduce the risks of rectal and advanced adenomas by about 40 percent.

"Among different cancers, colorectal cancer is the most influenced by diet," said study researcher Dr. Katharina Nimptsch. "Compared to something like smoking, diet is not a large cancer risk factor, but it does have an impact."

Previous research has found that a diet high in red and processed meat may increase risks of colon cancer. Other risk factors for developing colon cancer include heavy alcohol consumption, lack of exercise, as well as diabetes and a diet rich in fat.

However, previous studies have investigated diet during adulthood, rather than focusing on what people eat earlier in life, and their future cancer risk.

"Colorectal carcinogenesis is a long process that can take several decades, and the initial steps of carcinogenesis may occur at young ages," the researchers wrote in their study.

In the study, 19,771 women ages 34 to 51 answered questions about their diet during high school. Over the following 10 years, 1,494 of the women were diagnosed with colorectal adenomas. Of these adenomas, 305 were in an advanced stage.

"Our findings do not suggest an association between red meat intake during adolescence and colorectal adenomas later in life, but higher poultry intake during this time was associated with a lower risk of colorectal adenomas," the researchers said.

Eating more poultry and fish in adulthood didn't seem to change the risk, according to the study.

The researchers found it surprising that the results didn't show a link between red meat and the risk of adenomas, Nimptsch said. "When you look at the amount of meat people ate, it was really high." However, there were few women in the study who ate very little red meat; perhaps there was not enough variation to see a significant difference in risk, she said.

As with other observational studies, the findings cannot show a cause-and-effect relationship. "Before recommendations are made based on these findings, it is necessary that results are confirmed," Nimptsch said.

The results add to previous evidence that eating poultry may decrease risk of colon cancer. However, what mechanism might underlie the link remains unclear.

The study was published June 19 in the American Journal of Epidemiology.

Central bank group sees policy change challenge

BERLIN (AP) ? A body that represents many of the world's top central banks says policymakers should be mindful of the risks of maintaining ultra-loose monetary policies too long.

The Bank for International Settlements, a Switzerland-based organization, said in its annual report Sunday that changing course will raise "significant challenges."

It says central banks will have to strike a balance between the risks of switching policy prematurely and those associated with delay.

While the former are well understood, the BIS says it's important "not to be complacent" about the latter just because they have yet to materialize. Rising inflation is often identified a key risk of keeping policy loose for too long.

Evolutionary biologist Richard Dawkins introduced this year's Saatchi and Saatchi New Directors' showcase in Cannes last week, an event that focused on the theme of memes. ??

The choice was fitting since Dawkins coined the word "meme" in 1976, long before the word was adapted for the Internet age.?

With Hearing Implants, Experiencing Sound for the First Time

After being born severely hearing-impaired, Sarah Churman heard her first clear sound at age 29 and promptly burst into tears.

?I hear Melinda say, ?How does it sound?? Churman wrote in her memoir ?Powered On? about the experience. ?I start to answer her, and I realize I can hear the noises in my mouth. Then I realize how I sound. Then I get choked up. Then I laugh. Then that sends me into a fit of tears and choking up.?

Churman received cochlear implants at age 29. A video of her implants being turned on for the first time went viral in 2011.

Cochlear implants, which help people hear by electronically simulating the auditory nerve, have been used by 30,000 people worldwide, according to the American Speech-Language-Hearing Society.

With the spread of cochlear implants, videos of deaf people hearing for the first time have become a staple on such video sites as YouTube and Vimeo, garnering millions of views.

In the video shot by her husband, Churman lights up when she hears the noise for the first time and then quickly starts crying and laughing as she says, ?This is weird.?

But what is it really happening when people gain a new sense?

The cochlear implant is not a replacement for an ear, but it can help many people who were effectively declared deaf. By stimulating the auditory nerve, signals are transmitted to the brain, which turns into ?hearing.?

Although Churman wrote that she loved hearing and her cochlear implants, Dr. Daniel Lee, director of the Pediatric Ear, Hearing and Balance center at the Massachusetts Eye and Ear Infirmary, said many other adults sometimes describe the sound as metallic or robotic at first.

?[Usually you] are communicating through thousands of channels of information that are being sent to your brain,? said Lee. ?These implants have no more than 22 electrodes.?

Lee said that the implants are key for many children or infants born with severe hearing loss who receive the devices so that they can grow up with auditory skills and have stronger language skills.

Doctors, however, have to be careful when exposing small children to sound for the first time. Since the children often do not have the means to communicate, doctors have to be very careful that the children have a positive experience and aren?t frightened of their new sense.

Lee said the key is to keep the electronic data to a minimum in the beginning of using the implant.

?[You] don?t want to overwhelm them,? said Lee. ?Over a period of days to weeks to months [the implant] is slowly ramped up to provide more information to the ear or the brain.?

In videos usually a family member speaks to the child for the first time. In one memorable video an infant responds to hearing his mother?s voice ?by dropping his pacifier and looking at her in awe.

While cochlear implants have ?significantly helped people who were previously ?profoundly deaf, they do not work for people who either lacked an auditory nerve or had a damaged nerve.

But new technology is now being used that sends messages directly to the brain itself. A Food and Drug Administration clinical trial currently underway is looking at the effectiveness of electrical implants placed directly on the brain stem.

Placed directly on the brain stem, it bypasses all auditory nerves.

Grayson Clamp made headlines earlier this week for becoming the first child in the U.S. to receive the procedure.

Lee, who is involved in the trials but not in Grayson?s case, said that the auditory brain stem implants could be used to help deaf children communicate and develop lip reading skills. He said once children like Grayson are older, they can see how effective the devices are by evaluating their language skills.

But even before the implants can be measured and quantified, Grayson?s immediate response delighted his father, Lee Clamp.

?It was phenomenal to see him take that sound in and try to figure out what in the world is this? I?ve never had this sensation before,?? said Clamp.

NEW DELHI (AP) ? U.S. Secretary of State John Kerry will urge fast-growing India on Sunday to curb emissions that contribute to global warming and ease barriers to U.S. business and investment in the world's largest democracy that is viewed as a counterweight to China.

New Delhi is the second stop on Kerry's two-week visit to seven countries in the Mideast and Asia. During his stay, the top U.S. diplomat also is expected to discuss a myriad of other topics, including enhancing security in the region and prospects for finding a political resolution to the war in Afghanistan.

Kerry has traveled to India before, but his two-day visit is his first to the country as secretary of state. He is to deliver a speech that will focus on climate change along with other bilateral issues.

India has installed about 1,000 megawatts of solar power in the past 2 1/2 years ? about one fifth with American financing. Education also will figure prominently in their discussions. India is facing the challenge of educating about 500 million young people during the next decade or so and there is an opportunity for U.S. schools to help meet the demand.

Discussions also will include India's relationship with its archenemy, Pakistan, and the hope that Pakistan's new president, Nawaz Sharif, will try to improve relations, thus reducing the chance of a fourth major war between the nuclear-armed neighbors.

Kerry is talking about climate change just two days before President Barack Obama is to unveil his long-awaited national plan on the issue.

People consulting with White House officials on Obama's plan say they expect the president to put forth regulations on heat-trapping gases emitted by coal-fired power plants that are already running. Environmental groups have been pleading with Obama to take that step, but the administration has said it's focused first on controls on new power plants.

More than half of India's power comes from coal and while the U.S. has emission issues of its own, it wants to see India and other nations in the region rely less on old, coal generation facilities. The U.S. is backing a Turkmenistan-Afghanistan-Pakistan-India pipeline that would bring energy to a power-starved region.

Potentially life-saving cooling treatment rarely used for in-hospital cardiac arrests

June 21, 2013 ? The brain-preserving cooling treatment known as therapeutic hypothermia is rarely being used in patients who suffer cardiac arrest while in the hospital, despite its proven potential to improve survival and neurological function, researchers from the Perelman School of Medicine at the University of Pennsylvania report in the June issue of Critical Care Medicine. The authors suggest that scarce data about in-hospital cardiac arrest patients and guidelines that only call for health care providers to consider use of therapeutic hypothermia, rather than explicitly recommending it, may explain the study's results.

In a prospective study between 2003 and 2009 of over 530 hospitals in the United States, the Penn team found that 98 percent of over 67,000 patients who went into cardiac arrest in the hospital received only conventional post-resuscitation care--leaving just 2 percent who received therapeutic hypothermia, which has been credited with saving the lives of a growing number of patients who arrest outside hospitals.

"We know it's being used in patients who went into cardiac arrest in their homes, at work, or anywhere else outside of a hospital, but little was known about how often it's used in patients who arrest in the hospital," said Mark E. Mikkelsen, MD, MSCE, assistant professor in the division of Pulmonology, Critical Care and Allergy at Penn Medicine. "We found that even though most hospitals have the capability to treat these patients with therapeutic hypothermia, it's not being used. And even when it was used, in nearly half the cases, the correct target temperature was not being achieved.

"Several factors could explain this: there is little data, which is often conflicting, to support its use for patients in the hospital, and we have national guidelines that only have clinicians considering its use, which may lead to hesitation and lack of institutional protocol."

Cooling the body down to about 89.6 degrees after cardiac arrest protects it against neurological damage initiated by the lack of blood flow and oxygenation, several studies of out-of-hospital cardiac arrest patients have shown. It has also been shown to improve survival--a welcome development, since cardiac arrest survival statistics remain grim, with less than 10 percent of patients surviving in most cities across the U.S.

More than 300,000 people who go into cardiac arrest out of the hospital die each people each year in the United States; thousands of others are left neurologically devastated.

About 210,000 patients a year go into cardiac arrest while in the hospital--many of those patients may have other conditions that point to a poor prognosis, and a substantial portion may be terminally ill patients who are not candidates for hypothermia.

National recommendations established in 2005 call for out-of-hospital cardiac arrest patients to be treated with hypothermia when they remain comatose after resuscitation. In-hospital recommendations, however, are less direct. The International Liaison Committee on Resuscitation guidelines recommend providers to "consider its use," while the American Heart Association recommends that therapeutic hypothermia "may be considered" for a patient who goes into cardiac arrest caused by non-shockable rhythms.

For the study, the team analyzed treatments of 67,498 patients at 538 hospitals participating in the American Heart Association's Get With the Guidelines-Resuscitation database from 2003 to 2009. Of those patients, 1,367 patients were given therapeutic hypothermia. The use of therapeutic hypothermia increased slightly, from 0.7 percent in 2003 to 3.3 percent in 2009.

Younger patients and patients who were treated in a non-ICU location and a teaching hospital were more likely to get therapeutic hypothermia. Even when it was used, however, target temperature (32-34o Celsius, or 89.6-93.2 degrees Fahrenheit ) was not achieved in 44.3 percent of the patients within 24 hours, and 17.6 percent were overcooled.

"These rates are particularly important to examine, given that the incidence of in hospital events appears to be increasing," said Dr. Mikkelsen. "I believe there is potential for therapeutic hypothermia to benefit this population, but traction can only be made after clinical trials investigating safety and effectiveness are initiated-which are certainly warranted. Results of those studies could strengthen the case for stronger recommendations and increase use."
